I think there are a lot of people with mental illness, even people with conditions like NPD and APD who have difficulty empathizing with other people, who can attest that what Bella has done is an extreme that goes well beyond the boundries of what can be considered an "episode". She isn't some gibbering mental case (regardless of how she types) who is incapable of understanding concequences; quite the contrary, her actions following her being doxed have made it very clear that she's not only aware, but is fucking terrified of the concequences of the shit she's done, and is trying to pass the buck to Fiona or whomever else she can to worm away from it. She might not've believed she would've ever been caught and felt assured that she could get away with it endlessly, but arrogance is not the same thing as ignorance.
She was explicitly trying to get someone to kill themselves for fun and profit, allegedly raped (and even explicitly referred to it as "rape" in her little chats) and is documented on video sexually harassing a girl, and tortured animals to death for fun while snarfing their shit, all while talking about it with the little cult she was seemingly running. She had numerous people and authority figures looking into her shenanigans and she went out of her way to obfuscate the truth to avoid being held accountable.
These are the actions of someone who is living their fucked up wants vicariously while hoping that the "I'm so crazee!" defense will ensure they aren't held accountable for their actions. She even claimed to have autism at one point to try and guilt-trip one of her victims for fucksake. She knows that if she plays it off as a mental health issue then sympathizers will treat her with kid gloves or excuse what she's doing as just her being young and mentally unsound.
Frankly I'm not even convinced her parents knew about this shit; we have yet to hear from anyone (to my knowledge) who knew her before her college years. By the sounds of it this escalation from kind of smelly weirdo into full blown animal torturer and wannabee rapist/e-killer is a far more recent development that's been brought about since she's been let off the proverbial leash of her parents' control (or her nanny or whatever unlucky SOB they hired to take care of her while they were jetsetting around the world).
Tl;dr: Even if she does have mental health issues it wouldn't make what she's done worth anything other than contempt. You might as well be sympathizing with Jeffery Dahmer or the Green River Killer. They also started out as mentally fucked up teens who needed help, but that doesn't excuse what they became.
