The strait is very narrow, this means a lot of weapons - including cheap, small, and easily hidden ones; including ones that a single person could move, setup, and launch - can hit ships trying to go through the strait. When moving through the strait, ships don't have much room to maneuver, and ships must turn. That is, you can't just throttle to max and get a 50 mile running start so you're pushing through at top speed.
So if you have spotters down the channel, your strike team can be in position and have a sitting target.

The usual counters to missiles: anti-missile interceptors or CIWS-miniguns like Phallanx would have virtually no time to detact a launch and engage so escorts don't really work. now after the team launched, they would be utterly annihilated with overwhelming firepower, but it would be very, very difficult to find and ID any attackers before they launched. And Iranian Regime troops don't care about becoming martyrs.

Assuming that Iran had nukes and was attacked by a nuclear power, the counterfire would come from Iran. Unless their attacker could guarantee that Iran can't return fire with any of their nuclear missiles. I believe that is simply not possible given the size and geography of Iran. I've always wondered about the feasibility of a suitcase nuke because it's a completely different thing than a raped suicide bomber with a barrel of TNT in their ass. An actual suitcase nuke is theoretically possible but highly infeasible because the technological difficulty in building one makes it a pointless endeavor to pursue.

A 'dirty bomb' is not a nuclear weapon. It's a conventional weapon with radioactive materials. It would cause a huge explosion and the radiation emitted would make it deadlier than the conventional munitions alone but the amount of damage it can actually do is far below a nuclear warhead. Dirty bombs are possible but luckily, no one's attempted to detonate one yet so it's still theoretical speculation at this point.
The suitcase nuke is possible because as you state, Russia and America developed warheads that could fit in small spaces like a large suitcase but to go from that to what people are meaning when they say "suitcase nuke" is a stretch. The only countries that could pursue would have to be nuclear superpowers already and dedicating the money and time necessary to build it isn't worth when you already have ICBMs. It's not impossible. America or China could possibly build one but why?

The Iranians can hit Kharg Island with Shaheds that are launched from austere launch sites anywhere in Iran. They could target it with their shorter range missiles as well. I doubt they'd use artillery because that'd get hit withing minutes.

to stop iran from launching anything at the strait you would have to take and hold massive amounts of territory along the southern coast of iran

1. Divide the area within range into sections and have a few drones and loitering munitions patrolling each.(Optional)

2. When a launch is attempted tag launch site and people and track them until you send them a predator missile sandwich. Even if they get the missile or drone off they're still dead meat. No ground troops needed. This ain't the 70s anymore. We have a few more tools than Chinooks and infantry to flush out Charlie.

The only things I can find are 'a completely not public fatwa issued in the 90s', and a 2010 'prohibition on weapons of mass destruction' but I can't read Farsi, so I can't do much investigation into that.
