Assuming that Iran had nukes and was attacked by a nuclear power, the counterfire would come from Iran. Unless their attacker could guarantee that Iran can't return fire with any of their nuclear missiles. I believe that is simply not possible given the size and geography of Iran. There'd always be a risk of eating at least one nuke in return.
You kind of hit on it there. The Government of Lebanon is very weak. And has long been oppressed by the better armed Hezbolah thugs in the South. Israel is killing off Hezbolah.
