Her mind is not right. But you cannot expect much of an 80 year old who lives in utter filth and has no meaningful socialization. She does not look like she is going to keel over, but her mind is continuing its transformation to pudding.
Chris might not even realize that she is not quite right. He is a selfish autist who only cares about himself. He probably should not be recording stuff with her though. People will start reporting him for elder abuse again. Barb ending up in a home is the worst possible outcome for him because all the money would go to her new accommodations.

Barb looked haggard as hell. Like something out of Parkinson’s training video for nursing home staff.
The only problem is that she’s a wily ol’ Skeletor, and has been caught acting all pathetic and frail before in begging videos.
Eyewitness report and random pics (granted, from a year or two ago) certainly paint a different picture of Barb.
I guess time will prove whether we’re seeing Barb’s actual condition, or if it’s her standard begging face for whenever Chris makes videos.

Yeah but she wasn't asking for donations or anything in this video which was odd, just a "TELL THEM YOU'RE DOING OKAY BARBARA!!" from Chris who feels it's okay to order around his elderly mother.

Barb might not be the sharpest knife in the drawer. (even if she is the intellectual giant of the Chandler household.)
But she is a wily old hag, and intelligent to know that she can’t act feeble and frail in begging videos and then act normal in Chris’s other videos.

Probably doesn't qualify for hospice. But I'm wondering whether a medicaid accepting nursing home would be better then where she is.
And for those who say "anything is better than living with your gender-confused sonichu-fucking son"... I don't think Chris physically abuses her (I dunno I've been away from Chris content for a few months). There are some bad nursing homes out there. And you just know there would be no effort to put her in a good one.

Medicaid has asset rundown requirements before you're considered eligible. I don't know what it's like in Virginia specifically, but a lot of states cap that at around $10k which means you can't own a home. It's a liberal tactic to make sure wealth can't be passed on so that blacks have a chance to catch up and eventually enslave whites. I also don't know how debt is considered when calculating medicaid eligibility.
