Quick primer on PV. PV is a cancer of elevated red blood cells and platlets. There's 2 categories: Primary and Secondary. Primary is the typical mutations leading to cancer, while secondary means there's an underlying cause. I know it's been theorized that he's had a stroke in the past, but he's at high risk for a stroke because of this. It's also treatable with aspirin, weight loss, phlebotomy, and some select chemo agents. So to actually treat this, he needs to lose a good amount of weight. If he actually complies with treatment (fat chance), he might actually live another 20-30 years, but if he doesn't its about 1.5 to 13 years left to live.
