You either had transsexuals, which were primarily hsts, or transvestite/cross-dressers, which can now be called agps. This is pure speculation, but I suspect AGPTSs have always been the majority under the medical model of transsexualism from the very start, at least here in the states. Like you pointed out, AGPTSs just blended in better back then because they had to. All transsexuals back then were adept at lying to doctors to get what they wanted. A lot of HSTSs didn't even fit the classical transsexual archetype and were just sex workers who wanted hormones so they could easier prostitute themselves to straight men. Christine Jorgensen, the first widely known transsexual, was AGP, fwiw. If you're interested in learning just what a Wild West transsexualism was during its infancy, I recommend reading The Transsexual Phenomenon: A Counter-History and The Socio-Medical Construction of Transsexualism.
