The reason it happened is essentially because Israeli pilots were strained from constant sorties and the patrol map that noted where the Liberty was got wiped without the information carrying over.

LBJ didn't do anything wrong imo, he used the incident to press for a ceasefire. Initially it was thought that it was a Soviet attack and that's why the American planes that were sent out to protect the Liberty were recalled.
