I very rarely watch TV shows with a built-in narrative formula, but House is an exception for me simply because it's easily the one of the best to ever do it, it's hard carried by the fact that the characters are actually entertaining to watch and you know, DO THINGS.
Phineas and Ferb spends every other episode fucking with its formula, to the point where I think they've subverted it more times than they've played it straight.
I love house, but lets be real, its the absolute worse example of "The universe always resets for next week." House is banned from progressing or advancing as a character in any way and any progress made is reverted by the end of each episode because its a syndicated episodic procedual.
I never got that impression that scene was about trans whatever when I saw that scene. I just thought Ribbit putting her bow on Jax's head was her way of saying that she loved him and him blushing was his way of visually saying he loved her too. Him walking away from her out of personal anxiety right after and she abstracting was meant to be shocking.

All of that could work fine if handled competently. But if its all happening on the whim of an exec for marketing reasons it's naturally going to be terrible.
House is a very formulaic show that breaks formula maybe a dozen times for the entire run. Every episode starts with a cold open of the patient falling sick, then the patient gets transported to the hospital and the gang spends 2/3 of the episode trying to diagnose the patient, while the remaining 1/3 is spent on either melodramatic overarching plot or wacky House shenanigans.
It's like how the last few years because it became inappropriate to say "tranny chaser" they just say "chaser" and it's supposed to immediately mean a trannychaser.
