Generally theory is that he does/says creepy shit for attention which can be considered trolling. With that said between the psych evaluation and his actions (those fucking teeth, his cat pissing everywhere, his apartment being looked at by the landlord) his attention grabbing thoughts aren't too far from reality and he is fucked up.

Just a body pillow he likes to pretend is Anna. Kind of like a dakimakura but without the 2D waifu on the cover. He's had several actually, his aunt has thrown them out every she found them while Nick was away. Probably because they were covered in shit and jizz.
Though I have a pillow of Xion from Kingdom Hearts that I pretend is my oneitis Glenda. She kind of looks like Xion, doesn't she?

Nick's cat apparently has worms. He tried to get it in a carrier to take it to the vet, but the cat badly scratched him in the process. Given his hygene, he could get a nasty infection.

I don't know if he's making up crap to get a reaction here, but he claims he's considering "cutting her name" in his arm "again", as a "plan to obtain reciprocation". I'm pretty sure attracting women doesn't work that way.
