The true answer to your conundrum is to have the courage to stop lying to yourself. You are a religious Christian, a religion that takes pride in helping the weak and poor at great cost to yourself, and that clearly specifies that the wealthy are ngmi to Heaven. I am not motivated by "good deeds". I am motivated by duty. That's why I chose medicine. It's my way to serve my people. This is why I stay here and not leave for more money. This is why I want a socialized medical system where we get to fix even the homeless, even if that might mean wait times for the more wealthy.
Since I am basically socially reactionary, I don't feel that I should be compassionate towards some group or another. Again, it's not empathy that drives me, it's a pursuit of social order and peace. A largely harmonious society.
This is also why I live a modest life, in a tiny commie era apartment. It's why I walk or bike to work and to shop. It helps me stay fit and strong too. Car is for emergencies, and deathfats.
