Likely athletes foot. Apparently he wears those cheap, ill-fitting Wal Mart boots without socks. They're a fucking petri dish. It's also not going away/actively getting worse, something even minor cases of athletes foot will do if left untreated. He's also constantly touching it so clearly it's itching like hell.
