The comparison to Naoto from Persona 4 is very apt and the timing is such (2008 in the US) that it likely influenced Tom. IIRC Natani was introduced in the comic in 2006 and it was a few years until "zomg that one has TITS" was revealed. (As an aside, if you ask Google AI "when was she revealed to be female" it melts down trying not to answer you.)

Fun fact, and side note, they announced all of the voice actors for the new P4 remake EXCEPT Naoto, and the trannies have spent the past 18 years (well, probably a little less, since they likely didn't discover the game until they were told to give a shit about it) DEMANDING we all pretend that Naoto is a pooner. Thankfully, Tom Fischbach seems to be as resilient to drama as his brother and is smart enough to just ignore it. It has been relaunched ....at least twice by furries and the most recent one seems to go with a "Y2K" skeuomorphic appearance like its OG self and has clearly been made in the same cloth as Cara for being a "pro-human, anti-scalping, AI-free 'art' platform" but with extra flair of insanity like unique trigger warnings and actually attempting to prevent copy-pasting by using the "canvas" css code feature too. (Though inspect element exists to bypass this lol). It also has trigger warnings and customizable profiles, but also bad code oversights (like super long user statuses breaking the look by not having a wraparound or deliberate ellipse at the end of too long).

Seriously though it's so blatantly obvious that Fischbach's intent here wasn't transgenderism or even transvestism because he started this story LONG before that became mainstream, even for furries. These people just cannot take being told NO.
