Some of my favorites have already been mentioned. Here's a few that haven't.
One-offs:
Val's in Hayward, CA - Don't attempt the papa burger unless you actually do have a hollow leg.
Dutch Goose in Menlo Park, CA - Try the peanuts
Eagle's Nest in Cocoa Beach, FL - Also cheesesteaks. Usually really good, but last time I was there was not so good. Hopefully that was just a fluke.
The Hat in Alhambra, CA - Mmmmmmmm. Pastrami burger.
Helvetia Tavern possibly in Hillsboro?, OR - This is the big one. Seriously not sure if I don't want to keep this one to myself.
A few other places, but damned if I can remember the names. "That kick ass grill next to the watch shop a couple blocks up from the ferry terminals in Portland, Maine," probably isn't very helpful.
Chains:
White Spot in BC and Alberta are generally pretty good.
Steak 'n Shake in the southeast to midwest U.S. - I don't know why I like them, they're not super exceptional, but I just do.
Burgerville in the northwest U.S. can be really good.
Checkers seem to be popping up more and more places. Same with Johnny Rockets. Both are pretty good if you can tolerate the fake '50s kitsch.
Nathan's in the northeast U.S. - Yes, I know they're a hot dog chain. So what? I don't care. I like 'em.
The Hat is also a chain now. I saw another one in Pasadena recently. Fuck yeah! That can only be good. Hopefully they'll escape California soon.
