Did I read correctly that this wannabe fat actress spent 3 days in the hospital for an iron transfusion? And she identified one of her ”health conditions“ as “low ferritin”? Why is she complaining about months of being on NJ, baby food, formula and still looks so overweight?

Her writing is awful. And I don’t mean just piss-poor stylistically, I mean that she writes with many spelling errors and a surprising lack of intelligence for how she attempts to portray herself on her videos. I was caught off-guard by that. I suppose the one positive thing I can say about her is that she feigns more intelligent than she is, and in 10 second increments, is successful.

You just know her ER physicians hate her bullshit. I’m surprised she hasn’t been flagged with psych for these 3-10 day hospital stays with ahem unusual presentation of the illnesses she claims. She has to get smarter about medical presentation if she is going to continue this new actress role she has as a chronically ill patient.

Has she been flagged factitious disorder yet? She strikes me as someone who has a lot of amusing patient notes in her charts entered during ER stays. But she also strikes me as someone who drags mommy to the hospital administration with threats of lawsuits if she cannot see and edit her chart to remove things she doesn’t like, like “chronic bullshitter [sic].”

Yup, she spent 3 days in the hospital over an iron (and potassium) infusion. I guess that's what you get when you scream MCAS and anaphylactoid. Amazing how her MCAS reacts to things like her hair, the sun, and iron infusions, but not to the heavy makeup she occasionally wears.

Early on in her caring bridge, she mentions multiple psych visits and a doctor saying "it was all psychosomatic." While it's possible doctors stopped calling psych for her, I think it's more likely that she just stopped mentioning it. Her early psych mentions come across as "I'm such a victim because they don't believe me," but that isn't going to get you the sympathy you want when people are doubting your story in the first place. If she's been flagged as factitious, she hasn't said anything about it. Realistically though, when you're in the hospital for days with supposedly severe GI symptoms and no GI doctors come look at you, that says something on its own.

Any requests for an edit go into your medical record. If a patient is trying to edit objectively wrong information because a provider screwed up somewhere, that's fine. It's a good sign that the patient is proactive and trying to take charge of their own medical care.

She did supposedly have Mayo remove her FND diagnosis because supposedly doctors were treating her like a psych patient because of it and weren't taking her stiff person and EDS seriously. Although I somehow doubt the FND on her chart was the whole reason for that.

I liked the part where she was excited about getting an IBS diagnosis. IBS is one of those cOnDiShUnS that a doctor diagnoses to get you out of the office when all of the available testing has come back normal but you're still being obnoxious. She's acting like they've finally found her one of a kind genetic mutation or something. Actually, there are people who celebrate that kind of diagnosis less than Chelsea is celebrating not being able to poop normally.

I'm wondering specifically about Epic but I'm sure all of them have some nonpublished comment option.

Definitely trying to be read as blind with those dark sunglasses at night and the guide harness. Passed out and had seizures on a roller coaster. How she knows this when she was unconscious and her friend was not able to properly assess the situation due to being on a roller coaster is beyond me. This particular roller coaster is so indoor/outdoor with the indoor portions being in near-total darkness. There’s also TWO sections in said darkness where she’d be exposed to strobes, specifically: a photo spot at the end of a track switch, and the infamous “Disco Yeti.” The yeti animatronic that used to be the big finale of the coaster is broken in a way that will cause catastrophic failure of its support structure and potentially kill people if they keep running it, but it can’t be repaired without dismantling a large portion of the ride. Therefore it does not move but stands frozen with an arm extended while strobes flash to give the illusion of movement. It has been lovingly dubbed the Disco Yeti. Sydney had to be aware of it since she goes to the parks multiple times per week and it’s one of those weird little inside jokes.

After that video Syd takes a two month break from social media. The PT took it, walked away, and refused to fit her for the chair because Sydney doesn’t need it and the PT would be committing medical fraud by helping her obtain one. Sydney does not understand since this PT told her all she needed was a slip and her doctor has provided her with one. It could not be that the PT was saying she didn’t need a wheelchair and her doctor would have referred her for one if she did, who was then shocked that a doctor did refer her for one since she did not have any mobility issues. No, it was this dirty lying PT punching poor Sydney in the gut. This upset Sydney so much that she pretended to have a seizure, one of those ones where she’s fully aware that NO ONE EVEN CAME TO CHECK ON HER. This might be because they saw her filming and taking mirror selfies with the dog. She then ran up the stairs to be sick – recall that this was an assessment for a wheelchair she swears she needs – and passed out on the bathroom floor. PT lets her have her poopy pants tantrum for another half hour before coming to check on her (and tell her to GTFO). Finally, the PT keeps the wheelchair referral papers and won’t let Sydney have them back, while dialing up the doctor who gave it to tell on her. Absolutely based PT. This post went up the next day, when Sydney is fixing to march back and DEMAND her referral be returned, then she’s going to take it to the supplier herself and find a PT who isn’t an ableist meanie and will let her have her hard-earned prize! Based PT apparently did not return the referral, as Sydney did not get her custom ride. Oh god. It’s a triumph. A true work of art.

Aural hematomas are notorious for filling back up as soon as they’re drained which is why they “quilt” the stitches, and many vets won’t even bother to just drain and wait these days, and she has experience with what happens if you don’t treat these properly. So why, then, didn’t she go right to the quilting stitches? Because she thought she could cheap out and get away with just draining. What about "nonpublished comments" in the EMR? I only have access to the lab version of a chart, but I'm curious about this option. Do you know if "nonpublished" means the note never shows up in the patient EMR or is this just some weird lab thing?

I'm guessing it has to do with sequestration. Inpatient psych could read my notes but I could never read theirs (emergency.) Might be a way to keep things in-unit.

EPIC has a sticky note feature that only you can see. Since the CARES act we lost the ability for unpublished. I didn't heavily use them in MRI because most of what I am documentating is gado. Every now ans again if patient moved a lot I would flag the ordering and let them know they maybe some ativan next time.

Well that sucks. I always had standing orders to get them in the van if they got squirmy when they were rolled down to imaging (assuming no obvious contraindications like opioids or elevated BAC.)

I think a lot of hospitals have been telling providers to be "good stewards" of lorazepam though. Fuck you admin I'm sending every agitated patient on a ride.

IBS is one of those cOnDiShUnS that a doctor diagnoses to get you out of the office when all of the available testing has come back normal but you're still being obnoxious. She's acting like they've finally found her one of a kind genetic mutation or something. Actually, there are people who celebrate that kind of diagnosis less than Chelsea is celebrating not being able to poop normally.

The Epic version I'm familiar with shows EVERYTHING in MyChart, if that's what you mean. If anyone writes a note, the patient can see it. It's really not good for anyone, including the patient.

i also recall that she says she called and chewed out the neurologist at Mayo who told her she had fnd and not cci, but since the doctors don’t typically directly talk to the patients at Mayo (they’re pretty busy and have nurses) this seems unlikely. But who knows.

Now her tears and dramatics over removing her IV fluids during her attempt at a 14 day (12) NJ medical vacation at the hospital makes more sense. She knows that is the first step to getting someone ready for discharge and she wasn’t going to get what she wanted (until she threw a fit and demanded “nutrition” to get back to “her baseline.”

It doesn't. If you know screens and monitors induce seizures, you will limit how long you are on a computer or phone.
None of her seizure activity makes sense or is consistent. However, of it is convenient for her like the many times she uses strobe filters and flashes gifs at a high rate across the screen and doesn't seize. Or the many convenient seizures she has during testing.
 
Finally, the PT keeps the wheelchair referral papers and won’t let Sydney have them back, while dialing up the doctor who gave it to tell on her. Absolutely based PT.
I like how horrified the PT was that the dr. filled out the note. I feel bad for the dr. though bc now it makes him look bad that he prescribed it and she could get him in trouble. Meanwhile Sydney is trying to get the PT in trouble... 🙄

I totally agree with you regarding it not being great for patients to be able to see everything. It's like that old episode of Seinfeld where Elaine schemes to read her chart because she thinks there's something insulting in it. I think it makes providers resort to using euphemistic and coded language to get across their message about the patient, and that can be unclear. I think there are some subjective observations that need to be in the chart, but not all of them need to be visible to the patient. For people like Chelsea, it's especially important, because anything negative that a provider says about her is grounds for her to do that BPD "splitting" thing, which is a huge obstacle to treatment.

Risks like this seem to have gone largely ignored when the government decided to give patients access to their full medical chart.

I don't know if they should even be able to see subjective observations. They can't request to change them anyway. Even nice-sounding coded language will set a BPD munchie off, and then they're off to find another doctor.
