It's difficult to know if this was filmed before or after officially being under the care of that doctor, because the TikTok is controlled by orbiters/enablers. According to @hereforelvafrompl they get sent photos and videos, then they post it with made up captions and narratives, and they also edit them and use AI.
The stories they make up with these pictures are obviously contradictory, with some being about getting treatment and others being about fucking his face up with more crap. Many are obviously ragebaity, and about "stealing your man" or whatever. The reason people could believe it's not orbiters running it is that this person is obviously insane anyways, so contradictory and strange messages would be kind of expected. A drug addict could do the same.

It's honestly just morbidly fascinating to watch people slowly and painfully kill themselves for no sane reason. At least with a drug addict or alcoholic their form of slow suicide is relatable to a degree which makes it sad as I could see myself ending up at that same spot, someone like Elva on the other hand is engaging in behaviors that are so far outside the norm and with such disastrous results that you just want to keep watching and see what happens next.

I'm pretty sure there was also something in the deal like two mandatory psychiatric appointments after the surgery, which is obviously not a lot and just to say "we dealt with the immediate medical threat to his life and referred him to a mental professional for further treatment". I don't see how removing life threatening fillers is anything but helpful. It's non-cosmetic and everything he's said up until this point actually suggested that he was against reverting his "progress" so I think he views it as a medical issue as well, I don't have high hopes that he won't go back to doing the exact same thing that lead him here, but for the moment he seems fixated on other forms of self destruction so there is a non-zero chance that he actually stays away from the fillers for a bit.
