Nick's definitely got more staying power with his viewership. Nick's far better at maintaining his hetero-facade than Elijah. That's not to say that Nick is not an obvious flamer, just that Elijah is so much worse.
I don't know who the hell his audience is. He's been around forever and it's become clearer with his talk on his recent streams how he's managed to stick around in the periphery of the online right. Elijah's formation of and participation in the "Riot Squad" shows that Elijah has done a lot of networking and groundwork to maintain his position despite not really ever having any real popularity. I think he was able to keep some good relations after the allegations at The Blaze but once the divorce and Sarah Stock information was put out by Milo it seems everyone other than Fuentes has abandoned him. At this point I think the majority of his fans are groypers who watch it just because they watch everything in the groyper sphere.

Weren't they always a thing? For as long as I remember, right-wing women having a gay friend or whatever was... I don't want to say normal, but fairly common where I was. I can't speak for anyone, but it always seemed like their boyfriends/husbands were like "They're a novelty, she's curious." They'd never let them be alone or have a girls-only day or whatever, but seemed to be something to tolerate to keep the woman happy.
