Another update from the Tragic Thumb. Seems like there’s always something else going on with her. Now withdrawing from Zoloft.

So I’m supposed to get closed with a muscle flap today buuuut I’ve been in a lot of pain and running a fever of 102f they took more blood cultures and they know my white blood count has been up and my tsh was 17, also I’ve been without Zoloft since I got admitted to the hospital on the 3rd of November and have had withdrawal symptoms. I’m also hurting a lot in my ribs. I’m supposed to have surgery at 1pm today but idk what’s gonna happen since I’ve been running a fever, but I just wanted to give a brief update. Either way I’ll still be here until at least Friday or longer. Thanks so much to my family helping me through such a hard time idk what I would do without my sisters mostly. It was also mom’s 60th birthday on the 16th so I’ve been pretty depressed, not having Zoloft and it being Halloween and thanksgiving hasn’t helped either. Thanks so much for everyone’s kind words it’s meant a lot ❤️
From what I can see in the latest USPI for Zoloft, there are cardiovascular risks associated with Zoloft treatment:

  • Serotonin Syndrome: Increased risk when co-administered with other serotonergic agents (e.g., SSRI, SNRI, triptans), but also when taken alone. If it occurs, discontinue ZOLOFT and initiate supportive treatment. (5.2)
  • Increased Risk of Bleeding: Concomitant use of aspirin, nonsteroidal antiinflammatory drugs (NSAIDs), other antiplatelet drugs, warfarin, and other anticoagulants may increase this risk. (5.3)
  • Activation of Mania/Hypomania: Screen patients for bipolar disorder. (5.4)
  • Seizures: Use with caution in patients with seizure disorders. (5.6) Angle Closure Glaucoma: Avoid use of antidepressants, including ZOLOFT, in patients with untreated anatomically narrow angles. (5.7)

I can understand why they wouldn't want her to be on Zoloft post-bypass, considering the interactions with antiplatelet drugs, but why do they have her going cold turkey? Is there not an alternative to sertraline, or does she need to go through withdrawal before they can prescribe that?

I guess the reality is probably that Becky is conveniently not mentioning that they're managing her withdrawal for added pity.

The way she wrote it, it seems like she was admitted and didn’t bring any of her home meds with her. Although, knowing that the hospital would likely reconcile her med list, I’m surprised they aren’t giving the zoloft to her, or atleast not tapering down? It would be even more of a mess if she suffered complications due to her comorbidities (aka being on deaths literal door)

But this is also from the mouth of the Thumb, so I take any update with a grain of salt.

I mean, she didn't die of sepsis, which is where I would have previously guessed the start of the visit being, "Fever of 102, extreme pain, dehydrated because she only drinks full sugar soda," would end up. While I understand why people aren't impressed she's sick, she isn't dying. Sometimes, medicine means being presently surprised the patient got better. Whoever took care of Becky did a good job. It makes me glad to see people care about their patients, especially when they care about someone like Becky. Goals, or something.

it looks like a permanent down grade as the best case scenario to me, which means she ends up in the hospital a few times a year every year until a perfect storm of complications or aging takes her out. A worse scenario is that the perfect storm of problems is brewing up right now. Sometimes people completely change their lives after surviving something like this but they tend to be intelligent or at least high-agency people. Becky goes with the flow of life even when it means your blob-monster girlfriend makes you clean her butt rags, so I don't expect much.

the only things they can cross-taper to are other SSRIs and from what i can tell the cardiovascular risks are for that entire class of drugs. there are some other weird psych drugs out there for her to try later if she makes it through this, but most have weight gain as a side effect. her best bet is to regularly exercise once she is cleared for it and to do some cognitive behavioral therapy.

This is Amber's future and Chantal's present. Multiple organ early-failure makes it difficult or impossible to treat each individual disease with the best drugs.

I chatted with an ER doctor recently and asked what they'd do for emergency pain management or anesthesia in a patient with heart disease, kidney problems, diabetes and (presumed) fatty liver. He said there are entire classes of drugs they wouldn't use. They'd have to choose between a couple different opioids. They'd have to "go slow", meaning very low doses and then only increasing bit by bit as necessary because those drugs would still be bad news for her.

Wow, finally out of the hospital, albeit with a bunch of tubes attached still. I wonder how long she'll get those nurse visits.

I also wonder what her prognosis is based on her comorbidities and all the complications etc. It's been rough, and taken a long time. Picc line in for IV antibiotics until the end of December, really?

Big Al must be feeling some type of way situation type deal over Beggy being able to fish for venmos and asspats on the very platform Amber brought her to while she's broke and cryeeeen.
With all of the issues she has, if she was just a liiiiitle bit older, the hospital would’ve transitioned her to a skilled nursing facility upon discharge. Jesus.
 
You know what? I'm glad to hear Medicaid is covering an in-home nurse for her. She'll have someone (hopefully) competent 3 days a week and may give her some much needed advice/reality checks while she's there--within the scope of her capacity, of course.

And Becky will have the chance to interact with a stranger who has no fealty to her outside wound care and no skin in the hater game. Might help her outlook and give her a little perspective.

I had a conversation with an old man over Thanksgiving about all his conditions. He's basically like Becky, but he's 80 and not very overweight.

He said his doctors try to balance the medications for each condition, but they're frank with him that different meds are mildly damaging different organs. Something he was on for his cardiovascular system made his kidneys noticeably and permanently worse before the doctors caught it. He said his diet for diabetes is hard to balance with the kidney diet and watching his sodium.

Becky is in the same situation, but she's not willing to do the hard work to slow down the degradation of her organs. She'd probably eating Doritos with Mountain Dew as I type this. She'll end up on dialysis in a couple years, if her heart or an infection doesn't get her first. The 80 yr old man has decent odds he'll outlive her.

Gonna go out Chance Carmichael (500goingon600) style… in a hospital bed at home, PICC in place and loaded to the gills on narcotics.
Addiction, deterioration, death.
 
Just give the Thumb all the dilaudid and bowls of soup beans she wants. It'll keep her occupied, quiet, and out of everyone's hair for the last few months.

I was just thinking about her the other night. From the silence, I thought she was probably dead, which is probably how it's going to be every time she goes a week or so without posting. Then again, her family seems like the type to immediately announce her death so they can get going on the inevitable crowdfunding "for the funeral".
 
This thread reminds me of all the old people who subscribe to the newspaper just so they can check the obituaries every day to see if anyone they know died and don't read any other part of the paper.
