My recollection of the lore (I didn't read it recently) is it has no defined shape. Hence it is "Unknown". It's some malevolent force fueled by rumor and belief. Every town has its mysterious happenings and the town the map is based on had people spinning ideas of a crazy ax killer, government weapon experiments, and more. The tricky thing that makes it so cool is if you try and look into the exact cause of the disappearances, it manifests and tries to kill you. In this case the person the lore story follows was also confusing it for the entity, the scary thing that runs the game in-lore, and both sort of showed up to take her at the same time and the entity decided to take the creature instead.This kind of sucks the backstory of this killer is one of the better ones they have ever made an unknowable monster my best guess from what I’ve seen it’s like a parasite that hijacks the corpses of its victims and just twists them just a few notches away from being totally unrecognizable.
How the trannys can see themselves on a creature such as this baffles me
But again they have to make everything about them and their delusions
All that to say, it has no defined shape and is only vaguely humanoid for two reasons:
-It's informed by the rumors about it (it also has a gunt flap that launches hallucinogenic bombs as a nod to the government rumor) and it probably uses this shape to lure victims with how it also has a lot of lines asking for help.
-in a literal design sense the killer had to have some sort of kind of defined shape to make it consistent to recognize and face (and make skins). If it was different every time new players could get confused if it was like a weird dog, a human, a blob, etc each time.
So they decided to make it a weird deformed humanoid cryptid looking motherfucker and because it has no sex or actual permanent body in lore they made feminine skins to exemplify this.
The fact it makes a perfect tranny caricature is completely unintended and that's why it's so funny.
