So in answer to your question, does he think? No, not really. Not in the way normal people would recognize as thinking. If he did think (something that requires some degree of self-awareness and honesty) it would cause him terrible pain (narcissistic injury) and worse depression than he probably has now.

I have an online friend for many years, who I knew to be a lesbian, and would furiously defend herself as one. During the years she would occasionally talk about her “girlfriend” while joking about how gross bisexual women and trannies are.

Turns out, this “girlfriend” was a 40 year old, not-even-remotely passing agp male. My friend is in her early 20s.

I always knew trannies were delusional, but the partners of them are honestly just as bad. My man sounds like a gay male at best, and would unironically refer to himself as a girl and a lesbian. My friend would accommodate this delusion and use all the classic fetishy terms like “good girl” all while absolutely insisting she is 100% a true and honest homosexual, even when the reality of her situation was highlighted.

Honestly, I don't even think it's the pickle surprise is getting these people killed too often. I think that's just their weird fantasy way to go.
