In all honesty, it's kind of hard to gauge when you really start to think about it. For one, fandoms now a day tend to act as mental asylums for the most deranged in society and they're spread all over the god damn place. This is on top of actually trying to get a bearing on who's actually the worst. Let me give a few examples. Block games generally tend to have the worst e-dramas over the most meaningless and petty bullshit and their userbase tends to be incredibly vindictive. In that regard, block games have some of the most autistic fandoms in existence, but not the worst.

The most obnoxious fandoms are the [insert the current popular goyslop] type fandoms. Fandoms that attract tweens & teens. Fandoms that come to mind include the Amazing Digital Circus, Homestuck, Hetalia, Dream SMP, Hazbin Hotel, and half a hundred different fandoms that exist and existed come to mind in less than a Planck second. However their fandoms are just that, obnoxious and annoying. They'll irritate the absolute fuck out of you but they are mostly harmless with the exception of those enclaves of extremely deranged users that most even within said fandoms would rather avoid.

The worst fandoms in my time of observing miscellaneous fandoms are those that encourage immoral or deranged behavior at it's very core, and not as a side effect of deranged users no matter how small or large that quantity is. The number of fandoms that in this category are few and far between. The proshippers and the lolicon people by their nature of enabling pedophilia are 100% near the top of the list of being "the worst". If you count the zoosadists as a "fandom" or a fandom off chute, then they'd also be towards the top of the list.
