Someone else responded with the context, but I like the elf ear picture reply because lore-wise in Tolkien's LOTR series, evil forces cannot create new things, they can only twist existing things into horrendous forms. Orcs were once elves, but were tortured and mutilated until they became far-removed from their original species. In the same vein, the person replying to the op in the twitter thread is doing the "you will never be a real woman" meme, but with that concept.
It's such a nice comparison to trannies themselves.

Would a self-aware pooner confuse metoidioplasty and phalloplasty?
The text describes rotdog, but metoidioplasty is the one where they "release" the hypertrophied clitoris.
Obviously it has to be the latter for the pun, but they messed up.
