I have put some thought into it and realised that Ralph is so vindictive that he will do the opposite of what you advise him just to spite you.

Try to tell him that he has a lot to live for (a lie) and he could still turn things around (very unlikely) and he will surely kill himself to spite you.

But we need Ralph alive. We need the entertainment and him to declare that he will walk two miles a day and be healthy just to spite us all.

So apply this reverse psychology, the proper way to intervene, the right way to get him to fight on is to tell him,
