Loss of sight would either have no effect in his behavior or cause him to accelerate, no way it would lead to him slowing down. I'm not sure if it would lead to him thinking he's getting closer to attaining his dream look or if he would think he's not doing enough because he doesn't have his bloated fish face staring back at him.

I just got reminded of something I heard of that really makes addictions hard to kick. the ritual/ habit of preforming said thing not just the substance if its drugs/smoking etc. people will still vape/ smoke "nicotine free" juice or cigs. same goes with injections, it's a habit he has formed along with his other obsessions.
