This exactly. No other group could begin to be as terminally online, chronically unemployed, and autistically obsessed with their cause as troons. I had my first stage of MLD phalloplasty (phallus creation only) with Dr. Drew Marano on September 13th, 2024.

Sometimes an MLD graft is closed with a split-thickness skin graft from elsewhere on the body. To prepare for this, my left thigh was shaved and cleaned in case that skin would be needed to close my back. I was lucky to have primary (single line) closure. I had three jp drains following surgery. Two removed the day I was discharged from the hospital (Sep 19th) and the last on Sep 23rd. When I first woke up from surgery my left nipple was pretty badly displaced due to how much my skin had to be pulled for closure, but it’s already coming back.

MLD grafts often use the thoracodorsal nerve, a motor nerve in the latissimus dorsi. I was very lucky to have had not one, but three intercostal nerves (sensory nerves) that were viable for my team to harvest with the flap. I also received three cadaveric nerve grafts during surgery. Dr. Marano explained to me that intercostal nerves actually haven't been formally described for phalloplasty yet, but have for other MLD graft uses. Unfortunately, there was no way to tell prior to surgery what kind I'd have available, so I went under with the assumption that they would just use the motor nerve. The intercostal nerves can be quite thin in some people, so my team was going to determine if mine were viable or not during surgery. Turns out they were! I’m very excited about what this could mean for my sensation.

I stopped propping on Oct 4th. As of Oct 16th I was approved to hang and start scar care (minus the open bit on my back, of course). As of now (the 18th) I have no tactile/temperature/pain sensation in the penis itself, but very interestingly seem to be developing erogenous sensation already. I’m currently about 7”(18cm) long and 7.5”(19cm) around. I had a goal of 5.5”(14cm) long. I initially thought I would definitely reduce the length, but it’s grown on me. I may end up keeping as much length as I can. My mobility is slowly returning. I can wear normal shirts and pullover sweatshirts now, but for a bit I was only wearing button-ups and zip-ups like I did for top surgery. It had zero effect on the dexterity of my hand, just the range of motion and strength of my arm.

I have endless good things to say about Dr. Marano. There was a mishap with the first gyn he referred me to, but after raising my concerns he was removed from his and Dr. Keith’s team. He is a very compassionate and considerate doctor. You can tell he cares a lot for his patients and the wider transgender community.

Surgery was covered entirely by NJ Medicaid. I submitted for approval of a gender affirming left MLD phalloplasty and associated tissue arrangement. At the time of surgery I was 18 years old, 145lbs (~66kg, bmi 23), and 1 year and 2 months on testosterone.

Consultation: October 3rd, 2023; November 29th, 2023

Pre-op: August 14th, 2024

Surgery: September 13th, 2024
