I mean, not even that is necessary. A nail trim or file down and a nail scrubber would suffice. But do these people even own soap? Neglecting personal hygiene is actually a common manifestation of many mental issues, so I'm unsurprised to see it in troons to a disproportionate degree, that and sheer laziness. But as a general rule, if you can't wash filth off your nails and belly, you shouldn't be trusted to take care of a raw surgical site (especially one ON said belly that's probably touched by THOSE nails).
