They'd convert to a religion (you're talking to such a person, although not for transgenderism). These aren't the kinds of questions that secular anthropology can answer, because it's all fluid and speculative—and ultimately, atheistic epistemology and ethics are groundless: see the attached pdf.
Secular ethics are fine for generating post-hoc rationalizations for contemporary tastes and values ("you should do what I already think you should, because I made up a story about how cavemen evolved to do that thing that I think you should"), but they have nothing to offer people struggling with those values: "Ok, I can just say that the cavemen evolved to do things my way—and even if not, wouldn't that just make my conscience a vestigial organ? Why listen to it, then? Maybe I'm the next step in evolution. And why care what evolution says, anyhow? It's an impersonal random process. And if my mind is the product of impersonal random processes, what makes me think that it's composed in such a way that I can arrive at truth? And if I can't be confident of that, then how can I know that my mind 'evolved' in the first place? Or that I have a mind (Descartes contradicted himself on this when he admitted that he could be wrong about every math problem—and logic and math are derivable from one another, so where does he get the 'therefore' in 'I think, therefore I am')? Or that there's a world?"

Pretty much, yeah. Ancient cases of believing your sex can be changed. I wanna get to the root of that, unless we only took an issue with transgenderism when it became annoying.
I'm not saying it's a bad thing to look into, but you can't assume that they have the same causes just because there are external similarities. A lot of them had various philosophical or religious reasons for doing that. The hijra in India identify with the goddess Shakti, for example.

Again, you're making the assumption that transgenderism is
primarily a mental illness problem rather than a moral-spiritual problem. That assumption isn't borne out from how it actually spreads.
Why do you think trans people hate detransitioners so fucking deeply? Why do you think they hate “truscum” so fucking deeply? Because it acknowledges that many people transition due to not being right in the head, a fact that literally destroys transgender ideology.
It’s to the point where transgenders are hateful of the doctors that perform their surgeries, because they know any time they can just come out and say it how it is.
We only saw dips in trans ideology when we started pathologizing it. It literally just does not work. All the trans memes and boykisser shit becomes disturbing to the average person if it’s mental illness, they can’t support that. There is literally nowhere else for it to go after that.
Historically, you have it backwards. Pathologizing transgenderism and homosexuality was the first step towards pushing for normalization, as it de-emphasized the moral-spiritual component. They hate "truscum" because it's a step
backwards in
their own program. It's true that pushing for pathologization may have been a solid incrementalist move in the other direction, but it doesn't follow from that that it's the root of the problem for the majority of people who identify that way.
You can get a good idea of who's gonna troon based on who they surround themselves with, how they deal with attention, their attitude towards sex, and their general worldview. Mental illness definitely can factor in heavily, but the fact that you can make solid predictions based on the others means that it's not
primarily about the mental illness.
