Some 4k TV's run automatically in 30 Hz movie mode. Same happened with my 4k when I hooked up my laptop to it to use as a second monitor.

Go into the settings and see if there is a "gaming" mode. Color accuracy and resolution may suffer a bit, but the TV will force itself to output at 60 Hz

I have it set to game mode, still have screen tearing.

The base PS4 isn’t 4k compatible - natively. You need to upgrade to the Pro model.
