Syphilis can do this to a person, it's called Syphilitic Dementia. But it's pretty uncommon and it's unlikely Pixy has it. For one thing, a person has to contract syphilis and then not treat it. This isn't beyond Pixy's scope for poor life choices, but I doubt this has happened. Then you have to be in the unlucky 30% of syphilis sufferers who get third stage syphilis. This comes with brain damage and syphilitic dementia, but also comes with other fucked up side effects like the cartilage in the nose and the roof of the mouth will start to degrade and dissolve.

The biggest reason she most likely doesn't have third stage syphilis is because it only rears its head fifteen or twenty years after the initial infection. Since Pixy was patently NOT having sex at fifteen, there's very little chance she contracted syphilis then.

So yeah, she's going batshit loony, and it might not be totally organic craziness, but it's also probably not syphilitic dementia.

When I saw the new pictures of her vagina my first thought was it looks quite swollen.

Considering this upswing in paranoia/psychosis, and the gender fluid thing I don't think it would be too far fetched if Pixy was pinching, pulling, or otherwise assaulting her clit and labia. She also might not be doing that on purpose but maybe obsessivly trying to 'examine' herself or some such thing.

I just can't think of something, even a bad infection, that would cause that sort of swelling. Well, Paps CAN hurt if you're super nervous about it, or it's your first one being done. If Pixy is going through perimenopause at 30, something is very wrong with her endocrine system and her GYN should have referred her to a specialist. Perimenopause starts in your late forties if your endocrine system is average and healthy.

Also her clit is totally within the range of normal in terms of size, though I agree that everything looks puffy and inflamed, which is probably a combo platter of her terrible hygiene and her constant poking and prodding.

I agree that all of this is way beyond the scope of usual PT crazy, but honestly most of these symptoms can be attributed to menopause. It is completely possible that she is going through perimenopause, which is basically pre-menopause. It would account for: hair loss (scalp), irregular periods, fatigue, vaginal pain (during and outside of sex), worse PMS symptoms, hot flashes (higher body temps), enlarged clitoris, growth of excessive body hair, and the most important one in her case, it can intensify mental illness, ESPECIALLY previously untreated mental illness. On top of all of that, she is still in the age range where schizotypal mental illnesses start rearing their ugly head.

Obviously I don't know for sure, no one aside from a doctor who has fully examined her would know, but it sounds like she isn't very open or honest with her doctors anyway. I do wonder though if what she said about the OBGYN telling her that she was just getting old was actually the doctor trying to explain pre-menopause, but PT probably immediately shut down at the word "old".
