I'm not one to judge weird fetishes, but takonomi's page is pretty horrific. Last year when that guy went on a one-man crusade against anime, taxonomy made an imageset in their honor. He speaks decent English and provides English versions of a lot of his stuff.
Now why am I putting a perfectly nice tribute to victims of a mass shooting? Because every single other image on takonomi's page is hardcore fetish stuff with a focus on quad amputees. In fact, this tribute was in between the Japanese and English version of this gallery
Now, you can probably tell just from looking at the title that it's pretty deviant, but here's the thing... it's relatively tame. It's a masochistic girl whose teeth have been removed by her master (who is a dentist) celebrating her third year of slavery. But what's with the 2.1? Oh, this is the third chronological entry in a 4 part series, the other entries let us know that
And this is really a fairly tame imageset by his standards. At least she learns to accept her lot and none of her eyes were removed. Here's a set that is not his most extreme collection, but is highly illustrative of his usual work.

They're tools inside those game as part of mods who can remove limbs or lower height more than the game limit.
If you want to know, no; the games without mods haven't those tools. Only a real madman can create and upload in pixiv.

There's this one Japanese artist, who draws extreme Yaoi gore (including but not limited to cuntboy, amputation, cannibalism, mensuration, and crushing) art of......Liam Gallagher and Noel Gallagher from Oasis. Sometimes the Beatles and Devil may Cry too, but mostly Oasis.

I wanted to a post on BenwayMD. Presumably named for the deranged doctor from William S Burroughs' writings, BenwayMD is a Western artist from Pixiv who has a delightfully deranged imagination for some truly insane medical experiments. His art is all done in pencil/charcoal except for the (frequently quite long) expository text which is done with a typewriter font; his drawing is not amazing, it's really the ideas behind it that make it so… impactful. Unfortunately his Pixiv account is completely nuked and he had no presence on any other sites as far as I can tell and the only gallery that I was able to find (there are so many real doctors named Benway that make him hard to Google) is on exhentai (you can't use e-hentai). The person who collected this gallery seems to have mostly avoided his sicker stuff (or maybe I remember him being more hardcore). The themes that he especially likes are: breast modification (including stretching, removing, transplanting extra pairs, moving them, and expansion), forced incest (mother-daughter and sister-sister), scat, fuckhole mouths, and hole stretching. If you don't want to go through that gallery or don't have an exhentai account here's a sampling of its contents.
Experiment 136b is the most disturbing one in the gallery IMO.

You know what, I've decided to not even post this pic just in case. You can find it on his pixiv if you really want to know what it is, it's a recent piece and it's just as horrible as it sounds.
The 2 is not parody, by the way. It's the second Bukkake Baby (the first one didn't get a subtitle). The 5 in the lower right corner suggests that this is the fifth page in a comic book, I don't know if this is the case, but it's not out of the question. Talon51's Pixiv includes over a hundred pages of his hand-drawn comic Prince of Perversion. It comes in three 35-page issues. The first one starts out with the eponymous Prince of Perversion trying to resist his pedophilic desires and a demon deciding to use him. It ends with him raping a 6-year-old and going to hell with her to be trained by the demon. Part two starts from there, but ends with him moving on to raping toddlers to death before ultimately becoming a demon himself (symbolized by him becoming a tranny). Third one starts there, moves on to him traveling to other universes to take more victims, and finally ends with a legitimately cool twist.
An alternate universe version of his first victim gets thoroughly raped by him, then turns the tables on him, humiliates him, steals his demonic power, and rapes him for eternity
