Sometimes, it takes a little while for the liver to go full cirrhosis and for the jaundice to become visible depending on how much he continues to drink and his overall biological fortitude. It is something that doesn't happen overnight for some people. The liver can take a lot of abuse. Some people drink and pop pills for years and they don't see the end result of their damaging drug use until they are in the 60s.

That's if the infected foot stump doesn't take Brian out first. He is well on his way out. The damage has been done.
