Ok, this might just be me but he almost looks kind of normal in this picture (Doctor Who not withstanding). I mean, think about it:
-No terrible hat.
-No particularly weird clothing.
-It looks more like he has a normal beard than a neck beard.
-You can't see his horrible teeth. Oh Christ, those teeth.
If you had no knowledge of his misdeeds and his repulsive demeanor, one could be forgiven for thinking this is just a picture of some unremarkable dude.
I don't know why I'm analyzing this picture so intently
. Regardless of how normal he looks in any picture he's still like a real-life equivalent of Lester Ballard.

It's a fascinating study in contrasts. Even serial killers can look normal when they want to. (See,here's one shaking the hand of the First Lady of the United States.)
It's particularly horrifying when they're so normal outside. Dennis Rader worked for ADT for 14 years, the US Census Bureau, and his city's Code Enforcement Dept. IMO his complete 'normalness' made him one of the worst ever, bc there was no clue of warning for anyone, man woman or child.
i'm not calling Nick a serial killer. yet He looks normal when he wants to.

Lack of hygiene. He never explicitly said this, but I wouldn't doubt that he's afraid of razors almost as much as dentists. What I wonder is whether or not that's his actual complexion or the filth.
