Not sure he is balding, its an odd way to lose your hair. What are those little marks, are they sores, or has he tried to do something to his hair, one of those "get your hair back" scams you always see. Could it also be shaving nicks, if he is trying to give himself an under cut. Someone on reddit noticed what I noticed, he's got hair and then it just stops lmfao is he shaving the back of his neck? It looks as if he's balded on the direct back of his head and now mostly just has straggling strands hanging on for dear life and the dog ears on either side of his head. Basically, the ponytail represents a combover for the back of his head lol. That's my hypotheses at least.

Alternatively, because of his poor lifestyle choices and general lack of hygiene, could his body be dissolving/rotting? We know his teeth are in extremely poor condition and I've heard of infections impacting the entire body if given enough time and lack of medical care. Severely poor dental hygiene can become a very difficult case of gingivitis/periodontitis.
