Word and PDF versions attached. The Word version will likely display oddly if you're having trouble installing the fonts in the first place.

I've adjusted the formatting a little to more closely match the original, and recreated the table of contents as the font changes caused things to shift around.

Pro-tip for the future: using the "styles" makes changing stuff across the document easy, rather than leaving everything in the "normal" style and doing each formatting change by hand.
