It monitors the accounts of the followers. If the account that "follows" was recently created, or hasn't done any tweeting or retweeting after so long, it's pretty much considered a sockpuppet. It could be that the account is legit, but if they're not doing anything with their account, then they might as well be fake.
If you look through some of his followers, you'll see that many stopped tweeting years ago, yet for some reason came back just to follow the Gen Zed account.
