It's the lack of showers/Chris picking at his spots. Seriously, a friend of mine got really bad acne as a teen and he wouldn't leave them alone, the result was a lump like that on his forehead as his picked/popped spots got infected.

To me it looks like he did the stylized look where he does the mark several times. So it looks, idk, scary or something. Like the slender logo but as opposed to trying to look spooky he is just shit at it. At least that's how it looks on the "To the left" pic.
