There is a small percentage that does not but then ya know it depends if you take it personal and make it immediately awkward.

You do that only and ONLY if you are introduced to a larger group and it would take like 10 minutes to shake hands with every one.

Unless they are sick or have no hands, I shake hands. Usually go for a firm grip when it comes to men, along with a gentle and looser grip when it comes to women.
 
Direct and firm while looking them in the eyes- to let them know I could crush their hand, but choose not to. And to establish dominance, the last man to release the grip is hardcore af. We end up holding hand for the rest of the day because we ain't faggots who let go first.

That's how politicians are taught to shake hands, especially in crowds where they're shaking everyone's hands. You don't want someone to be able to grab your hand and hold you still. At best, they're wasting your time or make you look bad, but they could always be pulling you close for a stab or a gunshot.

You also just want to move on from one person in the line to the next quickly and get as many people as possible.

I shake hands on a weekly basis at church and at interviews, everywhere else I wait for the offer. I have a pretty firm grip for small hands because of it and get complimented for it a lot.
