From what I've observed, you pay someone for a design they made (however badly designed it might be) of some person/creature/whatever.

Actually, that would be a pretty badass video game enemy. Just seeing it creeps me out. Now, imagine it crawling towards you making hideous noises as you frantically fire at it with only 10% health left. And then you run out of bullets.
