His jowls are beginning to set in. Phil has never really trained for anything, not even personal health. A lifetime of eating like shit, drinking gin, playing video games, and no exercise will do that to a person. Keep in mind his idea of exercise is carrying a jug of milk and walking around the mall. 38 going on 65.

His face has been droopy as fuck for a long while already. When he did his IRL shtreem a couple summers ago (coincidentally around the same time and during internet outages preventing him from doing regular shtreems...), he showed how he brushes his grease mop every morning and peered into the souls of anybody who watched while his droopy face threatens to melt onto the counter.
