Saya no Uta, written by the Puella Magi Madoka Magica guy. It was a pretty famous shock VN back in the day. From what I recall: MC was a college medical(?) student that got into a car crash, parents died. He woke up screaming because the entire world and everyone in it looked like raw meat. He meets the exception when he was about to kill himself; an apparently normal loli girl who lives in the walls of hospital. She ends up beinga Lovecraftian alien parasite that instinctively replaces all life on the planet after learning to emulate the native species (although human culture trips her up because she learns you must love the person you mate with). They kill/mutilate/eat people together after some hesitation (effects him very little mostly because they are meat pillars that scream at him inhumanly) and then he get the one meaningful choice of the VN: She can fix his brain and dip on him or they can continue being together. Getting your brain fixed gets you shoved into a mental health facility for killing and cannibalizing one of your classmates, not being fixed you get the "good" ending where you and Saya get shot and killed but as her last act she gives birth to the aliens you imprinted on with her and the human replacement begins
