I imagine it would be from shock as the pool chemicals hit the neo-vagina.

I think it's just because they're worried about getting clocked for wearing a swimsuit.

What bothers the most me about this image is the artwork. That tubby whale depicted in this isn't even fat like those medieval cherubs where it at least has strong artistic form. This creature is lumpy and deformed like whoever made it is trying to make them look awful.

Even the quality of the art is bad. It looks like it took ten minutes to draw and color. Staring at that hand physically hurts.

It's this. Even if everybody is polite and minds their own business, a facial expression or something will trigger them. That's called being unsafe.
