Not every annoying troon, but a lot of them, yes.
While it does happen to almost every tranny on a lesser degree, The extreme cases are in the troons that get the full operation as opposed to the run-of-the-mill troon that just uses Estrogen.
The human body runs a relatively tight ship with sexual organs accounting for the primary production of testosterone in both sexes. When you remove the testes you remove a major part of what balances your endocrine system, almost solely relying on your adrenal glands to produce the testosterone you lack.

Testosterone and Estrogen are not just "Male Chemical" and "Female Chemical" they both need balance within your system for your body to maintain its normal routines without issue.
One thing you should keep note of is elderly troons. See how little of them there are.
This is not solely due to the 41-42% thing. They genuinely deteriorate from the inside out due to their body chemistry shifting dramatically, putting stress on their body and causing other health problems along the way. Taking Estrogen in a body that does not need it will of course cause negative side-effects, but it's the castration that really speeds them into becoming ghouls.
