I think he’s brain damaged at this point.

Current theory is that he's falling over in a drunken stupor and hurting himself.

However, since he got back from Florida he has been talking a lot about Adderall, and he was visibly more animated today than he usually is. There was also a moment where it seemed like he visibly ingested something pink off his thumb and rubbed his gums, which leads to the theory he's snorting crushed adderall.

He has historically stated he's a big fan of uppers (cocaine, specifically) but there has been speculation he dabbles with meth too. A symptom of amphetamine abuse is rubbing at the skin and open sores that come with that.

I don't know if adderall abuse can cause that kind of behaviour, he's got to be abusing a pretty high dose if that's the case. (TMI: I take adderall for ADHD and I've never felt compulsion to get nervous and twitchy like the behaviour he was exhibiting today).

He's on meth, 100%.
He's used meth before. I recall during Dick Masterson/ Juju the Cow's Road Rage event, one of Ralph's pay piggies said on livestream that he smoked meth with Ralph in an alley.
Meth is also relatively cheap and probably all he can afford since he spent all his money getting cucked in Florida.
Also I googled "signs someone is on meth" and Ralph fits a lot of the signs. Notably the sores and his hand movements. Dude is tweaking.

During the pilled-out Crazy Taxi days with Rozy in the back and the Oday Simpson monologue after Manduh left, his skin was visibly splotchy and fucked up. Even more so than now.

I feel like if it was some injury he got from faceplanting it'd be more severe? They look more like he's picked at his skin or gotten bitten by insects or something.
