The only thing that seems real about it is WS's yelling and CR's reaction to his yelling. I think CR may have just been surprised by how hard WS was going with the yelling and unsure how to react as he expected something more canned.

Rock was leaning forward and waiting on the hit. The hit itself was fake as fuck. There was obviously no physical contact (edit: I was wrong, there was minor physical contact). Rock's immediate reaction to the "hit" was fake as fuck. WS's smiling as he casually walked off stage (didn't seem amped up at all), fake as fuck.

Conclusion: Fake as fuck, but kudos to WS for going hard with the yelling portion. If not for that part, no one would even be considering that it could be real.

Also: WS hasn't sat front and center at an awards show in 20 fucking years

That could be why CR was able to take the slap by WS and just keep going with his schtick right up until WS doubled down with the verbals, at which point CR got a bit discombobulated and wandered off the script. Hence the "We are here... to give a documentary out" line when CR tries to bring the show back on-topic.

After watching several more times after posting, the biggest give away to me is how CR was leaning forward and bracing himself
