It's probably a combination of both. 2 of those characters to my knowledge are just considered cross dressers that didn't troon out but there is a increasing amount of actual trannies in anime lately

Well, it's a troon if you define a poor fellow that got without his consent brain swapped into the mauled body of his own sister (because his own body got melted by aliens) by some kind of schizo scientist a "troon". I'll admit I didn't read the continuation of the manga, so if there's extra revelations the joke is on me. But the guy is more a victim of horrific sci-fi horror surgery.
Also the guy gets raped in a rather uncomfortable scene in which the rapist makes a point he fucked his sister before, so he should simply shut up and enjoy the ride. Creepy stuff all around, truth be told.
The Usual Suspects kinda burst a blood vessel on the double whammy of medical horror/rape, because it's not good representation you see.

It's a combination of first world problems, and an inability to separate fantasy (such as drawings) and reality. There's also been enough documented cases of predators having anime in their porn stashes that they use in grooming that they'll just automatically assume anyone who so much as wears a Naruto tee in public is a pedo. Given the actual issues that still plague Japanese society when it comes to paraphilias that involves real children, it's just them barking up the wrong tree, and it might be on purpose and not out of ignorance.

Sometimes they do, sometimes they take them seriously, even when it is not the main purpose of the anime (yaoi,yuri,etc). In Psycho-pass for example, it is framed as if the lesbian couple got "cured" from latent criminality because of their love of each other.
