The real question for me is whether she's actually underestimating Becky here. It seems impossible, I know, but you're right that Becky must have known the shit-talking was happening (in fact, I'm sure it wasn't as much as a surprise to AL as she'd like us to believe, either). So either she's going to use this "revelation" to try to manipulate Becky by proving her devotion to her in spite of her mean old nasty family -- or Becky is already well aware of the situation and has been using AL far more cynically and blatantly than suspected. (To be clear, she's been using her regardless -- but to do so with the full awareness that her family actively loathes AL and trashes her whenever they don't have their hands out is a whole new level imo. She's not just spinelessly deciding not to "stir the pot" and ignore the tension; she's complicit in her family's exploitation of her "partner".)

I would guess because she normally lays in bed, rather than upright in a chair, so her feet are somewhat higher up/laid flat. When seated upright, it creates more pressure on the veins in her legs, causes blood to pool, and fluid to build up.
