Another factor in addition to the ones already mentioned is that most FTMs are young mentally ill and/or austists— who aren’t exactly known for their adherence to great hygiene standards. There’s a lot of aftercare needs to get scars to heal well (along with the genetic roulette) and some of these gals don’t appear up to that task.
Case in point Colt Seidman, who has some of the thickest, gnarliest keloids I’ve ever seen. After the surgery she didn’t properly clean her wounds, gained a ton of weight which stretched out the already damaged skin, etc.

The results look pretty much as horrific as I’d expect them to look. Scars are scars after all. But I think it’s a mixture of things.
There are several different procedures you can do. Which one they do is dependent on size, health and other things. Very few people qualify for the least invasive kind.
When they do the double incision one, they also REMOVE THE GODDAMN NIPPLES and either replace it with a tattooed nipple (Cause no one, clearly fucking no one could ever tell a tattooed nipple from a real one!) or do a whole nipple graft.
My skin literally crawled when I read up on these totally medically unnecessary procedures.
Oh yeah, as for why the results are so horrible, I think a factor is the kind of doctor they choose. Instead of going to a surgeon with lots of previous experience with mastectomies, lots of these yards probably go to special “gender clinics”. Where the doctor haven’t done hundreds of mastectomies, but is a plastic surgeon who decided to get in on a booming market.

There's a reason the smarter ftm troons bulk after surgery, if they can't get peri/keyhole.
I mean, it still looks fucked up, but the curvature of pectoral muscles helps somewhat hide the scar lines.
Granted, peri doesn't look "cis" either, largely because nipple size and placement aren't changed.
There are so few good, male looking top surgery results that I figure the prevalence of scars in art or whatever is a massive cope tbh. Even after years, the results still look off.

Despite what a lot of people would rather believe (that all ftms are uwu soft fakebois) T (and working out/bulking up) do a hell of a lot to mask more feminine attributes. It's harder to hide/undo what T has done than vice versa.

If you look closely you'll see tells, but most people will see shirtless, flat chest, beard, short hair, male clothes, and think "man". Assuming the ftm in question doesn't get the squeaky Aiden voice from T, even voice won't be a giveaway.
On the other hand, mtfs either look like bimbo blowup dolls or men in wigs.
