So if an ally used a pronoun they they thought was the correct one but was wrong, then by definition they made a mistake. The ally intended to use the correct one but was wrong. What else is there to call it?

Oh, I'm in full agreement there. I was thinking it basically was a no-win scenario. You either are openly transphobic, or if you make a mistake, you harbor a deep-seated transphobia. The only real way not to be transphobic is to be trans to begin with.
