I'm not sure if this Supreme Gentleman would qualify for the gold medal, but this quintessential school shooter type born a century too early made his pent up angst manifest in almost unprecedented displays of human psychopathy. I'm by absolutely no means a shrink, but it is in my opinion at least safe to assume that you may have an issue or two worth talking to a professional when the Waffen SS thinks you're a demented sick fuck.

On a more serious note, to answer this question we should define what's more evil: killing someone outright or make them live a life of suffering and trauma? Does the number of people affected count? If it does, how many people must be killed before they outweight the traumatized people in the "received evil scale" (or vice versa)? And so on.

A list of names comes to mind while weighting these options; most of them have already been mentioned in the thread. The real question is not "who" is evil, that's easy to assess (if we go with the standard definition of evil), but "how much" is evil?
 
It's a boring answer, but I think particularly destructive leaders qualify more than sadistic, psychosexual killers.

These people have the collectivized resources of their nation at their disposal, so they can order both war and genocide, and are equally as responsible when things take a turn for the worse and retribution is sought.

In 1984, Theresa lost it again on Suesan and stabbed her with a pair of scissors and Suesan again survived. Eventually, Suesan decided to move to Alaska and Theresa agreed with the condition that she remove the bullet lodged in her back so Suesan had no evidence against her. She couldn't get the bullet out so Theresa had her killed and burned because Suesan's botched operation got worse.
She killed Sheila after Suesan died by tying her up in a closet and leaving her to die by starvation and dehydration.

Life in prison was too good for this asshole.

Columbiner or not, you have to agree that Eric Harris was really, really fucked in the head.

He was the main guy behind the whole massacre, only getting Klebold (who is still a fuckwit) once he failed to manipulate anyone else into shooting up the school, and his diary showed how psychotic he was. You could say he was on par or even worse than the just as fucking tweaked out Elliot Rodger. You gotta be ballsy to make a DOOM level based on your school, manipulate your way out of community service and therapy, threaten to murder some guy over petty squabble and then commit one of the worst massacres that ended the 90’s for many alongside 9/11 in what some argue was a prolonged Neo-Nazi murder of the half-Jewish Klebold... only to be labeled a victim of bullying that all the other whiny brats can look up to so they can shoot up their school!

Dylan Klebold was bad as well because he did shoot up a fucking school and killed innocent people but there is the manipulation and the whole thing just being more of a prolonged suicide for him that doesn’t make him as fucked up as Harris by a tiny bit. Doesn’t mean I sympathize with him, though.

General Butt Naked AKA Joshua Milton Blahyi. This refined gentleman was a few decades ago a warlord in Liberia. His methods included wanton slaughter of civilians, regular human sacrfice and cannibalism of children and leading his troops to battle totally fucked up on drugs, booze and human hearts they had consumed, wearing nothing but shoes and drag-queen wigs. Although the scale of his atrocities is somewhat less dramatic than bigger names in the list, his style alone makes him the walking epitome of pure evil. He is still alive.

I wouldn't even consider those guys most evil. They commited horrid acts because they either had a "kill or be killed" mindset or simply were questing for power which is selfish but at least there's a goal.

The serial killers who just went for defenseless people and then painfully killed and raped are the most disturbing. Andrei Chikatilo, Albert Fish, Jeffrey Dahmer, Ted Bundy, etc. Just reading what they did is enough to suppress my appetite.
