Maybe he had the surgery down in Mexico? Taking a flight from Washington to there isn't that expensive (cheapest was around $250) and it would explain some of the weird things that just shouldn't happen in an American hospital

If he has a passport getting in and out of Mexico is really easy.
In fact mexico has a couple of doctors that do GRS, and the mexico transgender center has options for penile construction, intestine construction, and cosmetic construction. Their website is in english, and hell they even let you pay with PayPal.

Dude, no. Seriously.
Phil lives on SSDI (mental health tugboat), which means that even if he didn't blow it all on tattoos and takeout food and various trinkets that catch his fancy, he would still have no means to save up for GRS and the associated travel costs
anywhere.
And there is no "going out of network" for surgery, because Phil is not covered by private insurance. His health coverage is through the Oregon Health Plan, which is Oregon's version of Medicaid. Because, you know, he's on disability and living in subsidized housing, and incapable of getting or holding any kind of job. He's a public charity case, who by some perversion of the system is considered competent to manage his own affairs despite that being manifestly untrue. He is not buying plane tickets to Mexico, much less paying for major surgery out of pocket.
Any one of us who have actually followed Phil and this saga can tell you this.
He did have the surgery at OHSU in Portland; that much, we do know. But there's tons of details about his experience that are weird and wrong, so when he actually did turn up as a eunuch, with a brand new meathole between his legs, we were all taken aback. A lot of shit still doesn't make sense (including the ethics of performing GRS on a semi-exceptional autistic man with conspicuously poor hygeine who has made no effort to pass as a woman). But no, he did not go to fucking Mexico.
I wish we had more information than the photo — SRS is extremely invasive, and there's obviously going to be a lot of swelling and ugly healing. Are we seeing him neglect it already, or are we seeing something vaguely normal? Like "lol stupid tranny that's not a vagina" doesn't make sense yet if this is just part of the process.
It's never going to be a vagina, no matter how well it heals.
But given the grotesque state of neglect Phil has always subjected his body to, dating long before surgery, I would not be at all surprised that he is simply incapable of maintaining the level of cleanliness required to avoid infections.
This is a man who doesn't wipe his ass, whose fingernails are always black with grime, who doesn't bathe regularly, and that this is all normal for him is, apparently, part of his autism. He has significantly impaired executive functioning. And while he might be able to follow specific instructions on a post-op handout, if those instructions do not include the most basic, no-brainer hygienic practices, Phil might not even think to do them.
And I'm talking instructions like, "wash your hands thoroughly and scrub beneath your fingernails before touching the surgical site or handling supplies"; "always wipe thoroughly after defecating, wiping from front to back"; "shower every day, ensuring that the area around the surgical site is clean"; "always use a clean towel to dry it afterwards, and wear clean clothing"; "always sit on a clean, sanitary surface while performing any post-op wound care." It's all basic shit, but as we've seen, Phil has never done the most basic shit in his normal, pre-op life. He has no good habits when it comes to basic cleanliness, doesn't think about it, and has never found motivation to be clean despite the fact others find his stench disgusting and reject him (in part) over it.
So I can see him ending up with a massive infection, even if he gets autistically fixated on following the post-op care instructions to the letter, because if the post-op instructions don't specifically tell him to perform the most basic, elementary, obvious hygienic practices that any mentally-competent adult of even low-normal intelligence already understands, then he won't do them.
