This is more common than you think. When you go all in with facial femmenization surgery, so much tissue is rearranged. This results in scarring and inflammation which can often become uneven if you're pulling peoples face down to shave their brow bones while getting a nosejob and getting your cheekbones shaved.

Plastic surgery patients doesn't focus on the importance healing process process and tissue massage as much as I would like them to. You need to massage scars to break up the fibrotic scar tissue that puts tension on the incisions and results in lateral incisions getting pulled in more areas than others. So, even if you have a perfect incision and skillfully stitch it up, the final result can still look uneven and wonky because of scar tissue.

Most FFS' Ive seen have this post-op asymmetry in some form or another. The solution for this is to break up the surgery in parts to more carefully manage the scarring which is expensive, takes time and more demanding on the body to go under general anesthesia several times or go for a revision later. Neither are great options.
